This card comes with a photo of the Carer and NHS number/DOB /Name of the person cared for to help with identification on health emergencies. This card includes a QR code that directs them to the Website for validity.
Once purchased, we will require an email of a photo and information of the person cared for along with their diagnosis they wish to be on the card. proof of waiting list or diagnosis would be required.
